The Thermage procedure was developed to help the signs of aging skin.  It works by tightening tissue, softening wrinkles around the eyes, lifting heavy brows and redefining sagging cheek & jowl lines.  Thermage is the only non-invasive method FDA approved to tighten loose skin. It uses radiofrequency (RF) energy to uniformly heat the dermis (a deep layer of the skin) while cooling and protecting the skin’s surface (the epidermis). It causes immediate collagen contraction followed by new collagen production, which occurs during the 12 weeks following treatment.

Dr. Berkowitz offers patients medication and applies a numbing cream just prior to treatment to assure  comfort. Each treatment pulse first feels cool, followed by a brief hot sensation, followed by an immediate cooling.

A full face & neck treatment is likely to take about an hour, and is enough for most patients to enjoy good results.  However, some patients elect additional treatments to further enhance their results.

Some patients may have mild rosiness following treatment, which usually fades within just a couple of hours.  Heat-related complications can occur but are uncommon. Patients are able to resume normal activities quickly, but often need a good night's rest following Thermage because of the medications given prior to the treatment.  Also due to these medications, our patients do require a driver.  There is no special care needed after treatment; but, as a part of good skin care, sunscreen is recommended.

Because Thermage penetrates deep within the skin to cause collagen regrowth, immediate changes are not often seen.  Most patients notice results gradually over 2-6 months.  Recently published studies show that improvements continue for at least six months after a single treatment session. Other studies show that thermal collagen changes can last several years, depending on the rate of the aging process.  We do request Thermage patients have photos taken to document improvements 4 weeks, 2 months, and 3 months following treatment.

BOTOX, manufactured by Allergan, is a safe and diluted solution of Botulinum Toxin Type A.  Yes, this is the same substance that causes botulism poisoning, HOWEVER it is purified and extremely safe in the small doses used for cosmetic & therapeutic injections.  BOTOX is injected just below the skin’s surface to prevent muscles from contracting, which causes the appearance of wrinkles.


BOTOX Cosmetic was the name given to the BOTOX product in 2003 when it recieved a new FDA approval for the temporary reduction of wrinkles between the brows.  These products are identical in structure and injection technique.  Throughout this article, BOTOX will be used to describe both BOTOX and BOTOX Cosmetic.

Do treatments hurt?

Patients typically describe a small pinch.  No anesthesia is necessary, though application of an ice pack prior to the injections may provide added comfort.

 

Is this a permanent solution to wrinkle reduction?

Not really.  Botox is broken down by the body within 3 – 4 months.  Once metabolized, the results also fade.  Re-treatment with Botox can be performed several times each year safely, but will never permanently offer wrinkle reduction.  There are other procedures that offer longer relief from fine lines and wrinkles, though riskier to the patient and more costly initially. 

 

What are the possible side effects of Botox?

 

BOTOX enhances cosmetic surgery results.

Most patients having cosmetic eyelid, brow or cheek lifts or laser skin resurfacing benefit from a single Botox treatment just prior to or during surgery. Years of experience suggest, BOTOX:

  • better prepares the area being enhanced by weakening the surrounding structure
  • smooths fine lines, especially for laser resurfacing, allowing a more uniform treatment
  • promotes better healing by allowing the area to recover without the surrounding muscles pulling


Patients should not experience any permanent side effects.  Temporary side effects infrequently include headache, nausea, or drooping brow or eyelid.  Treatments of the forehead may prevent patients from frown-like expression.  Most side effects fade within the first couple of weeks and typically vanish within 3-4 months.

 

It is also possible to have bruising, swelling, or infection at the site of the injections.  All are extremely rare.

 

Is Botox FDA approved?

Yes.  Botox has been approved since 1989 for the treatment of medical conditions such as muscle spasms – particularly blepharospasm, a disorder that affects the eye.  This past year, Botox Cosmetic was approved for the cosmetic function of temporarily improving the appearance of moderate to severe frown lines between the eyes.

 

Once a substance receives FDA approval for any use, it is considered safe for other applications.  Therefore, it is legal to have been using Botox for cosmetic purposes though the FDA had not given its approval until recently.

 

How long has Dr. Berkowitz been using Botox and for what?

Dr. Berkowitz has been using Botox for nearly 10 years for the treatment of blepharospasm, as well as for wrinkle reduction and management of migraine and tension headaches.  Botox injections are performed in examination rooms.

 

How much do Botox injections cost?

Botox pricing varies depending on the area being treated and the patient's gender.  Single area treatments range from about $225 to lift a woman's downturned smile to $525 to raise a man's drooping brow.  Dr. Berkowitz can provide an accurate price following evaluation.

 

BOTOX for Migraine Headaches?


Despite the surplus of medications available to lessen the pain, decrease the frequency and shorten the duration of migraines, headaches are not often eliminated without interfering with life's usual activities.  In fact, the medications – taken daily with or without pain present - often impede upon life by leaving patients in a “fog”.

Dr. Mark Berkowitz, assisted by BOTOX, has improved the lives of many migraine sufferers.  Botox, a safe, diluted solution from the botulinum toxin, is injected into muscles to prevent certain muscles from receiving messages from the brain to contract (or spasm).  It is thought that certain facial or neck muscles may be contributing to the pain associated with migraine, and possibly tension, headaches.  When the muscle is, in essence, paralyzed by the Botox injection, it remains relaxed leaving the patient headache free.

 




Botox has been tested for many years and received FDA approval in 1989, though not for migraine headaches.  Botox remains in a patient’s system for at least 3 months, and treatments may be repeated indefinitely as Botox is considered extremely safe.  Though rare, side effects are temporary and generally fade within the first two weeks following treatment.

 

While most insurance companies do not pay for this procedure, some have.
